As we approach the end of the 2025–2026 school year, the Health Office would like to share several important reminders to help your child have a safe and successful start to the 2026–2027 school year.

Medical Documentation for the 2026–2027 School Year

If your child has a medical condition that requires accommodations, emergency care planning, or medication administration during the school day, updated medical documentation must be provided before the start of the new school year. This includes:

  • Updated healthcare provider documentation of the medical diagnosis

  • Medication Administration Orders for any medication that must be given at school

  • Updated Emergency Care Plans, if applicable

Providing this information before the start of school will help ensure that appropriate health services and accommodations are in place on the first day of attendance.

Students Entering Fifth Grade

New York State requires a physical examination for students entering fifth grade dated September 3, 2025 or after. Please schedule your child's physical examination with their healthcare provider and submit a copy to the school. For female students entering fifth grade, the physical examination should also include a scoliosis screening.

If a physical examination or required screening (scoliosis, hearing/vision), is not completed and documented by your child's healthcare provider, these screenings will be conducted at school in accordance with New York State requirements.

Students Entering Sixth Grade – Tdap Immunization Requirement

Students entering sixth grade must receive the required Tdap (Tetanus, Diphtheria, and Pertussis) booster immunization. Documentation of the immunization must be provided to the school within the first two weeks of the 2026–2027 school year.

Students who have not provided proof of the required Tdap immunization or a valid medical exemption within this timeframe will be excluded from school attendance until the required documentation is received, as required by New York State Public Health Law.
